Puffiness or swelling under the eye is caused by protruding fat pads that occur as a result of aging, genetics and lack of sleep. Lower eyelid surgery, also known as a lower eyelid tuck or lower lid blepharoplasty, removes the loose skin and puffy fat in the lower eyelids restoring youthfulness and vitality in this key facial area which can give you more confidence and boost your esteem.

Lower Eyelid Surgery?
You may be a candidate for lower eyelid surgery if you:
- Experience puffiness or “bags” in your lower eyelids

- Appear tired and older because of protruding fat and loose skin in your lower eyelids
- Feel that you could look better with the extra skin and fat removed from your eyelids

What Does Lower Eyelid Surgery involve?
YOUR LOWER EYELID SURGERY CONSULTATION
A consultation with Dr. Lo is required before your lower eyelid surgery. During this consultation, Dr. Lo will discuss your goals for the procedure, evaluate your situation, and offer recommendations. Many patients choose to have their upper and lower eyelid surgeries done at the same time.
WHAT TYPE OF ANESTHESIA IS USED FOR LOWER EYELID SURGERY?
You have a choice of anesthesia for your lower eyelid surgery.
- Local anesthesia: uses local anesthesia to numb the area, and you are completely awake
- Twilight sedation (most popular option): uses local anesthesia to numb the area, and you are made comfortable with intravenous medications
- General anesthesia: you are completely asleep
HOW LONG DOES LOWER EYELID SURGERY TAKE?
A lower eyelid tuck takes about 30 to 45 minutes to complete.
WHAT SHOULD I EXPECT FOR RECOVERY AFTER LOWER EYELID SURGERY?
When you go home after the lower eyelid procedure, you should rest and elevate your head. You will have small Steri-Strips™ (thin medical tape) on the outer part of your incisions. Placing ice or ice packs on the lower eyelids will minimize the swelling and bruising. Dr. Lo has a special protocol to minimize your swelling and bruising so that you may return to normal activities quickly.
You may be able to drive in three to five days, and you may resume exercise in 10 to 14 days. However, you will need to avoid heavy lifting and exertion for two weeks.
WHAT TYPES OF LOWER EYELID SURGERY ARE PERFORMED BY DR. LO?
TRADITIONAL LOWER EYELID INCISION
This incision hides naturally below the eyelashes of your lower eyelid and in the crow’s feet. The resulting scar usually heals so well that you can barely see it. There is no damage to your eyelashes, and you do not lose any eyelashes from the surgery.
The excess puffy fat is trimmed and removed from the three fat compartments in the lower eyelid.
TRANSCONJUNCTIVAL LOWER EYELID INCISION
There is the option of placing the incision on the inside of the lower eyelid. This is called the transconjunctival approach. Dr. Lo may recommend this technique in situations where an individual has excess fat pads in the lower eyelids but minimal loose skin.
WHAT SHOULD I EXPECT WITH LOWER EYELID SURGERY?

LOWER BLEPHAROPLASTY RESULTS?
You will notice improvement immediately but swelling and bruising may hide the final results for many weeks. At one month, you usually begin to see the early results of your surgery, but it may take six months or more for all residual swelling to resolve so you can see your final results.
You will have regular follow-up appointments with Dr. Lo to monitor your healing for up to one year after the surgery.
HOW WILL MY
LOWER EYELID SCARS HEAL?
Your lower eyelid scars usually heal very well such that you may barely see them. Early after surgery, once the dissolvable stitches fall out, the incision or scar may be pink in color. As they fade with time, over the next few months, you may treat with scar creams and cover them with make-up.
HOW LONG WILL MY
LOWER EYELID SURGERY RESULTS LAST?
Typically, after Dr. Lo’s technique, you should not need to have your lower eyelid surgery done again.
WHAT PROCEDURES ARE USUALLY COMMONLY COMBINED WITH LOWER EYELID SURGERY?
DO I NEED AN UPPER EYELID LIFT WITH MY LOWER EYELID SURGERY?
During your consultation, Dr. Lo will evaluate your situation and determine if you need an upper eyelid lift with your lower eyelid tuck. If you have loose skin over your upper eyelid, then Dr. Lo will likely recommend an upper eyelid lift with your lower eyelid surgery to give you the best look possible.
DO I NEED A BROW LIFT WITH MY LOWER EYELID SURGERY?
During your consultation, Dr. Lo will evaluate your situation and determine if he would recommend a brow lift (forehead lift) with your lower eyelid tuck. A brow lift can correct drooping eyebrows and ‘hooding over the eyes’ by repositioning the brows to a normal, lifted position. The result is a more alert, rested, and youthful appearance that looks natural and not over-pulled.
WHAT OTHER PROCEDURES CAN BE COMBINED WITH LOWER EYELID SURGERY?
A lower eyelid tuck can be performed alone, or it can be performed alongside another face procedure to enhance the quality of your results. In addition to upper eyelid surgery and brow lift surgery, common procedures combined with lower eyelid surgery include:

Will I LOOK DIFFERENT AFTER LOWER EYELID SURGERY?
The Lower eyelid surgery is done very naturally, and Dr. Lo will not change the shape of your eyes. By removing loose skin and fat, you will look refreshed and less tired.

IS LOWER EYELID SURGERY PAINFUL?
Lower eyelid surgery is tolerated well and not usually painful. Most of Dr. Lo’s patients take extra strength TYLENOL® afterward or take no pain medications at all. However, if you need stronger pain medications, Dr. Lo will write you a prescription.

HOW MUCH TIME DO I NEED TO TAKE OFF FROM WORK ?
You should take about one week off work, as you may have bruising and swelling. You may put makeup on in about five to seven days to cover any bruising. You may wear sunglasses when you go out.

WHAT KIND OF STITCHES ARE USED FOR LOWER EYELID SURGERY?
Dissolvable sutures are used, meaning the stitches do not need to be manually removed; they will dissolve on their own in one to two weeks.

WHAT ARE THE RISKS OF LOWER EYELID SURGERY?
Lower eyelid surgery is safe, but there are risks with any surgery. There are the usual risks of bleeding, infection, healing problems, and dry eyes. Dr. Lo will discuss with you the possible risks in your situation.

WHERE IS THE LOWER EYELID SURGERY PERFORMED?
Dr. Lo performs lower eyelid surgery with general anesthesia or twilight sedation in the safety and sterility of Pennsylvania Hospital Tuttleman Surgery Center, Pennsylvania Hospital or an accredited . It is an outpatient surgery, and you go home the same day as your procedure.
You also have the option of having your lower eyelid surgery performed under local anesthesia in the office. Dr. Lo will discuss with you what he feels is best for your situation.
