Patients who are most satisfied with their plastic surgery have realistic expectations. What exactly does this mean? It means defining your goals for plastic surgery. It means understanding what can be corrected, what might be corrected and what will NOT be corrected. Plastic surgery is about improvement not necessarily complete correction. Good to very good results should always be achievable in the hands of a good plastic surgeon. Excellent results should often be achieved. Patient satisfaction with their plastic surgery outcome should approach 95- 98 %. Very occassionally a second procedure may be required. Women often ask me; which is better? saline or silicone breast implants? There often is no right or wrong answer. It depends on the woman’s goals and her breast anatomy. Many factors are considered such as amount of breast tissue coverage, skin quality, size desired and incision or scar placement. Only about 10% of women do I recommend strongly saline or silicone implants. The other 80% of women have a choice of which implant they prefer. Patients often ask me what can they do to improve their face when they turn 30 or 40. By far the best surgery is upper and lower eyelid surgery. Often around age 30-40, loose skin begins to gather on the upper eyelid. This can make you appear tired and older. The extra skin on the upper eyelids can also feel heavy and smudge your makeup. If you have bags under your eyes or puffiness, then a lower lid surgery can smooth the lower eyelid and make you appear refreshed and younger. Today, I saw a patient for breast implants that told me she wanted 450cc implants. I explained to her that I would try but the size of the implant used would depend on many factors that are determined before the surgery and during the breast implant surgery. This would include her anatomy, breast tissue, skin tone, breast measurements and intraoperative sizing just to name a few considerations. She was not happy. I told her that she may well look great in 450cc implants but I would determine that intraoperatively but I could not guarantee 450cc. I told her she may be best at 425 or even 475. Still she was not happy. I explained my 17 years of experience with breast augmentation and giving my patients the proper size that looks natural and beautiful. Today, a grateful and happy breast augmentation patient sent me an article on a woman’s experience with breast implant surgery called “My life in Implants” by Joyce Maynard published in More magazine Oct 2008 issue. This article deals with her experience with a total of 3 breast implant surgeries. My conclusion from this article, is that if you wish to have breast implants, go to a reputable plastic surgeon and do not go too big in size. The author’s problem is basically related to having too large breast implants for her body and the plastic surgeon agreeing to do this. Some women can have large breast implants and others cannot. Many patients ask if smart lipo is better? So far, based on the research there is no proof that smart lipo is better. It might have less bruising and swelling but this has not been proven. Our national societies are conducting research to answer these questions. Right now, I use ultrasonic liposuction and Power Assisted liposuction to give excellent results in the right patient. Okay, the economy is bad. Good people are losing jobs,houses etc. What about plastic surgery? Its simple, plastic and cosmetic surgery is a luxury item. I always advise my patients that if they cannot afford plastic surgery, they should wait; save up their money. If you really want something, you will find a way to get it done and once you save up the money and have plastic surgery it will be much more rewarding to you. For women that have asked, I do perform transaxillary breast augmentation with silicone
implants. However there are strict criteria for a woman to be a good candidate for the procedure. She must have a large enough armpit area for the incision or scar which is longer than if saline
implants are used. The maximum size of the silicone implant that can be placed through the tranaxillary approach is about 275cc. This means the final result will be about a ‘B’ cup size. A larger size is really not possible. If you wish a larger size, than the crease incision or the periareolar incision would be recommended. If you have loose skin in your stomach area or tummy, then the only way to improve this is to cut out the skin, or have a tummy tuck.
If you your stomach bulges because your rectus abdominus muscles are separated, usually from having children, then the muscles need to be put sewn back in the middle with a tummy tuck.
