The most important decision for a woman considering breast augmentation is whether to choose silicone gel or saline implants. I will summarize the important differences. When you look at a woman with breast implants, you cannot tell whether she has saline or silicone just by looking at her breasts. If you touch her breasts, you may be able to tell the difference. Saline implants are soft but silicone implants tend to be softer especially if the woman started with very little breast tissue to begin with. If the woman had a reasonable amount of breast tissue and skin tone, it probably will not make much difference whether she chose silicone or saline implants. If you have very little breast tissue, thin skin and almost flat chested, silicone implants will most likely give you a better result and feel softer and have much less chance of wrinkling and rippling. Saline implants may be inserted through smaller incisions(cuts) where silicone implants require larger incisions(cuts). Therefore breast augmentation with silicone implants leave longer scars. For silicone implants, I use the crease incision or the periareolar incision, occassionally I use the transaxillary approach. With saline implants, I prefer a small incision in the armpit which heals beautifully and no one will see. I also use the crease or periareolar incision for saline implants.
