Summers almost over

Its been a hot and sunny summer. So what’s new in plastic surgery??
Well, Dysport has been out for a while now, the new Botox competitor, but still not a clear advantage over Botox. Botox is still the winner for now.
Injectables such as Radiesse, Restylane and Juvederm have found their niche for patients and are well established.
Breast surgery, augmentation and implants, not much new to report. Still working on fat transfers, but the amount of augmentation is still small about 275 ccs or so, a B cup is the largest size.
Still, rhinoplasty, facelifts and tummy tucks and liposuction have been popular over the summer. Gynecomastia correction as well. Everyone trying to get procedures done before school and work.

Breast Augmentation

As a part of the breast augmentation consulatation we use the Vectra 3D imaging system. We like to show patients that there will be upper fullness due to the peck muscle being lifted. Women often think of the lower fullness and don’t realize there is fullness between the collar bone and nipple. We show them on the Vectra what that would look like so they understand the changes that will occur in their breast and appearance.

We provide patients with bras for post operative recovery. It is often thought that the number in the size changes but that is not the case. The number stays the same. It is the cup size that changes. For example, a women who is 36B has surgery she may be a 36D after surgery.

This is a busy time of year for plastic surgery. Many people are taking charge and doing surgery to improve what has always bothered them. Honesty about your Medical and Surgical History

On one of the forms that new patients are required to fill out before their plastic surgery consultation with me, asks about the medical and surgical history. This is the most important form because it give me information about whether the patient is fit for a plastic surgery procedure and to avoid and anticipate problems.

Most patients are very good at filling out their history. I can tell whether it has been filled out properly or not. I always ask followup questions about the answers. Obviously, some patients forget their medical or surgical history or are nervous and details are easily forgotten.

However, there are some patients that (believe or not) intentionally don’t tell me they had previous plastic surgery on their nose or face or state they never had liposuction. Meanwhile, being a plastic surgeon, I can obviously tell whether you have had plastic surgery before.

Honesty is the best policy. Your plastic surgeon is your advocate and friend. He/she needs all the information about your medical and surgical history in order to give you the best recommendations. This also includes telling your plastic surgeon about all your medications, herbal medications, vitamins, etc.

This will allow your potential plastic surgery procedure to go as smoothly as possible.

Copying Celebrity Looks

Patients will often bring in pictures of their favorite actor/actress from magazines and show them to me during the plastic surgery consultation. “Dr. Lo, can I look something like this? I really like this nose.”  ” I like these breasts on this movie star, I want my breast augmentation surgery do look like this.”

Most patients are smart and I encourage pictures to be used as a point of discussion. By understanding what a patient wants and their goals, then I can deliver surgically what they are looking for. Using the pictures, I can also tell the patients what is NOT possible and be realistic about what their goals are for their facial, rhinoplasty or breast augmentation surgery.  So, the bottom line is that pictures are useful when used correctly and patients are realistic.

What's new and on the horizon for plastic surgery

Patients often come in for their plastic surgery visit and ask” Dr. Lo, what’s new?”

Okay, let me run down a few items.  this is not meant to be a comprehensive list.

1. Dermal Fillers- the latest is Evolence; however  in the crowded dermal filler choices, I do not feel that it holds much advantage over what I am using now such as Restylane, Radiesse and Juvederm. Dermal fillers come in different consistencies depending on what areas you want to fill in and last about 6 months to 1 year.

2. Reloxin- This is the recently FDA approved alternative to Botox but is not yet available. We are still trying to determine what is the cost, and I think after that, we can determine if it will be better or more cost effective than Botox.

3. Liposuction- still the tumescent technique with ultrasonic or PAL(power assist) lipo are good techniques. We are waiting for the studies on Smart Lipo or Laser Lipo to see if it is proven to be better.

4. Breast Augmentation- not much new here. The new silicone implants are working well. For saline breast augmentation, I still prefer the Moderate Plus Profile implant. No word on FDA approval for the next generation Cohesive Gel implant although I believe these implants will have limited use as they are shaped, textured, require a  larger incision/scar and firmer than the current silicone implants.

5.Nose Surgery- The non surgical rhinoplasty  has limited application and is not permanent. If you really have concerns about your nose, than a formal closed or open rhinoplasty is still necessary for best cosmetic results.

6. Tummy tucks- no major change here. The pain pump has really helped patients recover faster with less pain and  back to work in 7-10 days.

7. Male chest contouring for gynecomastia- No change here. The combination of liposuction with subcutaneous mastectomy where necessary are still giving very nice results.

Realistic Expectations

As patients, we all want the best result, kind of like a Michelangelo portrait, but most of us understand that we have to be realistic. During the consultation, whether it be for breast augmentation, tummy tuck, rhinoplasty, upper eyelid surgery, liposuction or any plastic surgery procedure, it is important to discuss your goals with your plastic surgeon. It is even more important for the plastic surgeon to tell you what is possible, and what is NOT possible in terms of the outcome and result. This way, as a patient, you can make an educated decision whether you wish to proceed with the surgery based on what you want to achieve. Only a real and honest discussion with your plastic surgeon will allow you to closer to your goals for plastic surgery. Not too late to have plastic surgery

July 2nd, 2009

Patients often ask me if it is too late in the summer to have plastic surgery. Most procedures that I do, the recovery is 1-2 weeks. Whether it is breast augmentation, liposuction, rhinoplasty or tummy tuck,  with the short recovery times,  you may be back to the beach or swimming pool in 2 weeks. Your incisions will be healed such  that the water will not harm it. Now, you won’t be playing beach volleyball or doing laps or body surfing but the sun and water will not hurt you. (Always wear sun block, no tanning salons for 1 month.

Plastic Surgery Open House
